Indiana Home Care Guide (“we,” “us”) is an independent Indiana home-care information website by HomeCare-Force. This policy explains how we handle information collected through this website.

When you use the “get matched” form, we collect the fields you submit:

• Name
• Phone number
• Email address
• Indiana county
• Who needs care
• Type of help requested
• How you plan to pay
• How soon you need help
• Your consent to share the above with home care agencies

We do not collect medical records, Social Security numbers, diagnosis lists, or insurance member IDs through this form. Please do not include sensitive health details in free-text fields if any are added later.

Our hosting provider and analytics (if enabled later) may receive standard technical data such as IP address, browser type, and pages visited.

How we use and share lead information

We use match-form submissions to email or otherwise deliver your request to Indiana Home Care Guide and to home care agencies so they can contact you about services. By checking the consent box, you instruct us to share your submitted contact and care-preference details with agencies for that purpose.

We may also use the information to follow up with you, improve the site, or maintain basic records of inquiries. We do not sell personal information as a data broker product, but sharing leads with agencies is the core purpose of the match form.

Email delivery

When configured, form submissions are emailed using a transactional email provider (Resend) to an address we control. If email is not configured, submissions may be logged on the server for manual follow-up in development or fallback mode.

Cookies

The site may use essential cookies required for security and operation. We do not require an account to use the guides or form.

Retention

Lead information is retained as long as needed to fulfill matching, respond to inquiries, resolve disputes, and meet legal obligations. You may email a deletion request to the operator using the contact method published when available; we will process reasonable requests subject to legitimate business and legal needs.

Children

This site is intended for adults arranging care. We do not knowingly collect information from children under 13.

Changes

We may update this policy. The “last updated” date at the top will change when we do. Continued use of the site after changes means you accept the revised policy.
